Baby Circumcision Aftercare: The First 24 Hours

Stage 3 of the Circumcision Process

The first 24 hours following your baby’s circumcision are crucial for proper wound clotting, pain management, and initial healing. While the day is usually straightforward, your baby may feel more unsettled, tired, or irritable than usual as the topical local anaesthetic wears off.

Please read these clinical instructions carefully, follow the tailored advice provided by Dr Martin Harris, and keep this guide handy during day one of recovery.

1. Nappy Check Schedule & Nappy Changes

Close observation during the first few hours ensures that the surgical site remains secure and free of active bleeding.

Immediate Post-Procedure Nappy Schedule:

  • Initial Checks: Check your baby’s nappy at 30, 60, and 90 minutes after the procedure to verify that there is no fresh bleeding and that the dressing remains securely in place.

  • The First Nappy Change: Change the initial nappy after approximately 4 hours, unless it becomes heavily soiled sooner.

  • Ongoing Nappy Changes: After the first change, it is best to change your baby’s nappy right before each feed.

How to Fit the Nappy:

  • Use standard disposable nappies.

  • Fasten the nappy loosely to prevent any friction, rubbing, or pressure against the circumcision site.

  • Do not use double nappies.

2. Managing the Circular Dressing

A specialized circular dressing is applied around the circumcision site during the procedure. This dressing plays a vital role in protecting the delicate wound, preventing infection, and promoting blood clotting.

  • Do Not Remove or Adjust: Please do not touch, adjust, loosen, or remove the circular dressing. Dr Harris will inspect and change this dressing personally during your follow-up check.

  • Gauze Swab Top Layer: You will be provided with a small pack of sterile gauze swabs. If the outermost gauze swab becomes damp or soiled, you may gently lift and replace only the top swab according to Dr Harris’s instructions—leaving the underlying circular dressing completely undisturbed.

  • Hygiene Protection: The dressing shields the wound. Soiling on the outside of the nappy does not mean the surgical site itself has been contaminated.

3. What is Normal After the Procedure?

Understanding what normal healing looks like during the first 24 hours helps relieve unnecessary anxiety:

  • Pink Discolouration on Nappy: Small specks of blood or a pale pink stain on the dressing/nappy are very common in the first 24 hours. This occurs when urine mixes with small amounts of dried blood from the procedure and is not a sign of active bleeding.

  • Mild Swelling & Bruising: Minor swelling, redness, or slight bruising around the penis is a normal response to surgery.

  • Behavioral Changes: Your baby may be more sleepy, clingy, or fussy during nappy changes as the local anaesthetic gradually wears off.

4. Feeding, Comfort & Safe Sleeping

Keeping your baby calm, well-hydrated, and comfortable is essential for a smooth recovery.

  • On-Demand Feeding: Feed your baby whenever they show hunger cues, regardless of their usual routine. Regular breast or bottle feeds are the best way to soothe your infant and ensure normal wet nappies.

  • Gentle Handling & Winding: Cuddle and hold your baby as normal. When winding, hold them gently over your shoulder, taking care to avoid any pressure or friction against the nappy area.

  • Safer Sleep Advice: Always place your baby to sleep on their back on a firm, flat mattress. Never leave your baby sleeping on their side or front.

5. Post-Procedure Pain Relief (Analgesia)

Your baby’s comfort is our priority. If pain relief is required:

  • Follow Specific Prescribed Doses: Dr Harris will confirm whether your baby requires liquid paracetamol and will specify the exact product, dose, timing, and maximum 24-hour limit tailored to your baby’s age and weight.

  • Medication Safety: Only administer medications specifically recommended by Dr Harris. If you are ever unsure about timing or dosages, contact Dr Harris before giving the dose.

  • No Unapproved Creams: Do not apply any creams, powders, antiseptics, or ointments to the area during the first 24 hours unless explicitly instructed by Dr Harris.

6. What to Do in Case of Fresh Bleeding

Active, fresh bleeding is uncommon after circumcision, but it requires immediate, structured attention if it occurs.

If you see active, fresh dripping blood or blood soaking through the dressing:

  1. Apply Direct Pressure: Place a clean, sterile gauze swab firmly over the bleeding area.

  2. Hold Firmly Without Lifting: Maintain continuous, direct pressure. Do not lift the gauze repeatedly to check if bleeding has stopped, as lifting breaks the forming blood clot.

  3. Contact Dr Harris Immediately: Call Dr Harris on 07896 718812.

Emergency Warning: If bleeding is heavy, does not stop despite firm pressure, or if your baby appears unusually pale, floppy, unresponsive, or cold, seek emergency medical assistance immediately (call 999 or go to the nearest A&E).

Can my baby feed normally after the procedure?
Yes. Your baby can usually feed as normal after the procedure. Feeding soon afterwards may help to comfort and settle your baby.

 

Continue offering breast or bottle feeds regularly. Good feeding and regular wet nappies are reassuring signs during recovery.

How should I hold and dress my baby after circumcision?
Handle and cuddle your baby as normal, while avoiding unnecessary pressure or rubbing around the penis and nappy area. Loose, comfortable clothing may help reduce friction.

 

Fasten nappies slightly more loosely than usual and change them promptly when wet or soiled. This helps keep the area clean and reduces rubbing while the wound is healing.

What pain relief can I give my baby?
Dr Harris will provide specific advice about pain relief, including whether liquid paracetamol is appropriate and the correct dose for your baby’s age and weight. Only give medication as instructed by Dr Harris or a pharmacist.

 

Do not give additional medicines, creams or topical products to the circumcision area unless you have been advised to do so.

 

What should I expect during the first few days?
It is common for babies to be more unsettled, sleepy or irritable during the first day or two. Mild swelling, redness, bruising and a small amount of spotting on the nappy can also occur.

 

As healing progresses, a pale yellow, cream or beige coating may develop over the circumcision area. This is commonly part of normal healing and does not necessarily mean there is an infection. Dr Harris will explain the expected appearance of the wound and how long healing is likely to take.

 

How long does healing take?
The initial swelling and redness should gradually improve over the first several days. Complete healing can take a number of weeks. Dr Harris will advise you about your baby’s expected recovery and any planned follow-up.

 

When should I contact Dr Harris or seek medical advice?
Please contact Dr Harris promptly if you have any concerns about your baby’s recovery. Seek advice if you notice:

  • Bleeding that does not stop with gentle, direct pressure as instructed
  • Increasing swelling, redness or tenderness
  • Thick yellow or green discharge, a foul smell, or a wound that appears to be opening
  • Ongoing distress or pain that does not improve with the recommended pain relief
  • Poor feeding, fewer wet nappies, vomiting, unusual sleepiness or difficulty waking your baby
  • Difficulty passing urine or no wet nappy within the timeframe advised by Dr Harris
